Choose the authorization related to the care. Approved member, provider and service information forms the starting point.
Review actual diagnoses, services, service dates, units and charges. An authorization describes approved care; the claim must reflect actual care.
Check the billing and rendering providers, service location, modifiers and applicable professional or facility details. Complete any missing claim information.
Review the final claim before submitting. Retain the claim number and follow requests for information. A received claim has not necessarily been adjudicated or paid.
Multiple claims can be linked to an authorization. Review the remaining quantity and avoid duplicate submissions. Authorization alone does not guarantee payment.
